- Participation in SADHA, local component, and/or TDHA activities
- Recommendation form from SADHA advisor or other educator or TDHA or local component mentor

- Open to all SADHA students
- Service activities in school, community and/or faith-based entities
- Essay from student about their service participation
- One letter of reference from an educator or someone outside the school setting
- Verification of good academic standing from the SADHA advisor

Qualifications:
- Open to all SADHA students who have completed one year of dental hygiene school
- Academic excellence as verified by school transcript with a GPA of 3.5 or higher
- Participation in SADHA, local component or TDHA activities
- Service activities in school, community or faith based entities
- Letter of recommendation from a dental hygiene educator
- One page essay on future of dental hygiene
- Winner must be present at SADHA Awards ceremony
